2. Checklist: Documents Required Before Filling the Form
Before you download and fill out the CMRF Application Form PDF, ensure you have the following documents ready. Incomplete applications are the most common reason for rejection or delay.
- Medical Documents:
- Original diagnosis report or bed head ticket summary.
- Estimate of treatment cost from the hospital (on hospital letterhead).
- Referral letter (if applicable) from a government hospital recommending the treatment.
- Identity Proof:
- Aadhaar Card (Mandatory).
- Ration Card (Food Security Card) or Voter ID.
- Financial Proof:
- White Ration Card (indicating Below Poverty Line status).
- Salary certificate (for Above Poverty Line cases, usually to show financial inability to pay).
- Hospital Details:
- The hospital must usually be a recognized government hospital or a corporate hospital empaneled under government schemes (like Aarogyasri).
- Bank Details:
- Cancelled cheque or copy of the first page of the applicant’s bank passbook (The sanction amount is usually credited directly to the hospital or the patient's bank account).
